The conch piercing has moved from specialty to mainstream over the past few years, driven largely by its versatility — it accommodates both rings and studs, sits prominently enough to anchor an ear composition, and heals with reasonable predictability when the fundamentals are correct. Search interest in conch piercing has risen steadily, and so has the number of people attempting it at home without the context that professional piercers carry automatically.
Most online guides cover the basics: what gauge to use, how to clean it, what to expect during healing. What they tend to leave out is the anatomy-specific knowledge, the equipment requirements, and the failure modes that experienced piercers learn from watching hundreds of conch piercings heal — and sometimes not heal.

Inner Conch vs. Outer Conch: They Are Not the Same Piercing
The conch is a broad term that actually describes two distinct placements with different anatomical characteristics, jewelry compatibility, and healing profiles.
The inner conch sits in the large, curved bowl of cartilage at the center of the ear — the deepest portion of the ear's cartilage structure before the ear canal begins. It is pierced with a straight hollow-bore needle and wears either a flat-back labret stud or, in a fully healed piercing, a ring. The ring option in an inner conch is the dramatic, statement-making look that dominates curated ear content — a large-diameter ring that loops through the conch and frames the entire ear.
The outer conch sits on the flatter cartilage surface between the antihelix and the helix rim — the area between the inner bowl and the outer edge of the ear. It is typically placed to wear a stud and sits at a shallower angle than the inner conch. The outer conch integrates more quietly into an ear composition and is sometimes described as a "flat" piercing by piercers who prefer anatomical precision in their terminology.
Before planning a conch piercing, identify which of these two placements you are targeting. The placement affects the needle angle, the initial jewelry selection, the healing position, and the long-term jewelry options.
The Equipment That Makes the Difference
A conch piercing requires a longer needle than most people expect. The cartilage of the conch is thick — typically 3 to 5 millimeters — and the needle must pass cleanly through that tissue and exit far enough to allow jewelry transfer without the needle retracting before the exchange is complete. The gauge for a conch piercing is most commonly 16g or 14g. Fourteen gauge is the professional preference for inner conch placements intended to wear a ring long-term — a larger gauge channel supports the ring's movement without thinning the tissue channel over time. Sixteen gauge is appropriate for stud placements and outer conch piercings where the lower jewelry load makes the smaller channel viable.
A receiving tube is strongly recommended for a conch piercing. The conch's cartilage is thick enough that freehanding the needle through it while managing the exit angle is difficult without a guide. The receiving tube positions at the exit point, gives the needle a defined target, and protects the tissue on the exit side from unnecessary trauma.
Anatomy Assessment Before You Mark Anything
Not every conch is equally accessible for piercing, and the anatomy varies meaningfully between individuals. Before marking a placement, assess the following:
Bowl depth. The inner conch bowl should be deep enough to seat a flat-back labret stud without the disc backing pressing against the canal wall. Ears with very shallow conch bowls have less usable tissue for an inner conch placement. If the bowl is shallow, an outer conch placement may be more appropriate.
Cartilage thickness. Thicker cartilage takes longer to heal and requires more care with initial jewelry length — too short a post and the disc backing embeds as swelling develops. Assess thickness by gently pressing the cartilage between thumb and forefinger. Very thick, rigid cartilage at a given position is a signal to use longer initial jewelry and expect a longer healing timeline.
Antihelix positioning. For an outer conch, the placement needs to sit in the flat area between the antihelix ridge and the helix rim. If the antihelix is particularly prominent, the flat area available for an outer conch placement is narrower, affecting where the piercing can go.
Marking the Placement
For an inner conch, the standard placement is in the center of the bowl, positioned so that the jewelry will not press against the canal wall when worn. Using a skin-safe marker or a toothpick dipped in eyeliner, mark the entry point on the outer face of the ear and the corresponding exit point on the inner face.
Check the alignment by holding the ear up to a light source. You should be able to see both marks and confirm they are directly opposite each other on a perpendicular axis to the ear's flat surface. An angled placement — where the entry and exit points are not aligned perpendicularly — produces a piercing that wears crooked jewelry and may lead to migration.
For an outer conch, the placement mark should sit in the flat zone identified during your anatomy assessment, positioned far enough from the antihelix ridge that the jewelry will not press against it and far enough from the helix rim that there is adequate tissue on all sides.
The Piercing Process

Position yourself in front of a well-lit mirror or, ideally, have a second person assist — a conch piercing through thick cartilage in your own ear is one of the more technically demanding self-piercing procedures because the angle is not natural and stabilizing the ear while pushing the needle requires deliberate positioning.
If working alone, brace the elbow of your dominant hand against a fixed surface to stabilize the approach angle. Use your non-dominant hand to hold the receiving tube in position at the exit mark, pressing gently against the inner face of the ear.
Align the needle tip with the entry mark. Take a slow breath in. On the exhale, push the needle through the cartilage with steady, continuous pressure. Cartilage piercing requires more force than soft tissue — the resistance is significant, particularly in thicker cartilage — and hesitating mid-push creates unnecessary trauma. Commit to a smooth, deliberate pass.
When the needle contacts the receiving tube, it is through. Thread the initial jewelry onto the needle end and push the needle forward while pulling the jewelry into the channel. Close the jewelry and remove the receiving tube.
Initial Jewelry Selection
For an inner conch piercing, a flat-back titanium labret stud with a 6mm to 8mm post is appropriate for most initial placements. The post length accommodates initial swelling without being so long that the stud catches on everything. Once fully healed — which for a conch piercing takes 6 to 12 months — the post can be downsized and a ring installed if desired.
For a ring in the initial placement: most professional piercers advise against starting a conch with a ring because rings move more than studs and movement disrupts the healing channel. If you intend to wear a ring long-term, start with a stud and transition at full healing.
The Healing Reality for Conch Piercings
Conch piercings have a longer healing profile than lobe or helix piercings, primarily because the tissue is avascular cartilage with a limited blood supply. Initial healing takes 3 to 6 months. During this period, sleeping on the piercing is the most common cause of extended healing timelines and irritation bumps. A travel neck pillow — the U-shaped kind used on flights — allows side sleeping with the ear suspended in the center hole, eliminating direct pressure. This single adjustment resolves the majority of sleep-related irritation issues in cartilage piercings.
Clean twice daily with sterile saline wound wash. Do not rotate the jewelry. Do not apply any product other than the saline — no tea tree oil, no alcohol, no antibiotic ointment. All of these either damage the tissue or trap moisture in the channel, both of which slow healing.
